Train with the country’s leading provider of outcomes-centered primary care and behavioral health for people residing in long-term care.
Eventus WholeHealth provides care to long-term care (LTC) communities with a focus on medically vulnerable adults. These individuals often face multiple chronic conditions, cognitive decline, and other complex health challenges that require coordinated, consistent, and compassionate care. By embedding clinicians directly within LTC facilities, Eventus helps ensure continuity of care for residents who are among the most underserved populations in healthcare.
Eventus operates on an integrated, outcomes-centered care philosophy. Care teams are multidisciplinary, often including primary care providers, behavioral health specialists, and nurse practitioners, working collaboratively to support the whole person. The approach emphasizes:
This philosophy ensures students experience first-hand how team-based, outcomes-centered healthcare improves quality of life in LTC environments.
The student clinical experience through Eventus WholeHealth takes place directly in long-term care (LTC) and skilled nursing facility (SNF) communities where students work alongside experienced clinicians.
By training in these settings, students gain deep insight into the continuum of care in skilled nursing and assisted living communities, preparing them for careers in primary and specialty care for vulnerable adult populations
At Eventus, students will experience firsthand our integrated care model, which combines primary care and behavioral health into a single, coordinated plan of care. This means you won’t observe episodic reactive care; you’ll see proactive, prevention-oriented care designed to:
By working within this model, you’ll gain unique insight into how clinical teams collaborate across disciplines to deliver consistent, patient-centered outcomes.
Your clinical training will take place in long-term care communities, where students are paired with Eventus clinicians. These settings include:
This holistic view allows students to see what it means to deliver outcomes-centered healthcare in long-term care blending disciplines, preventing crises, and always centering the needs of the resident.
Eventus WholeHealth students are immersed in a truly interdisciplinary learning environment, where collaboration across specialties is the norm. This structure allows learners to understand how different disciplines come together to deliver comprehensive, outcomes-driven care in long-term care communities.
Eventus WholeHealth provides services across a broad regional footprint, creating opportunities for student placements in multiple states and settings. This reach allows universities to match students with locations that are logistically feasible while ensuring consistent exposure to the Eventus care model.
Eventus currently operates across Indiana, Ohio, Kentucky, Virginia, West Virginia, North Carolina, and South Carolina supporting tens of thousands of patients annually in long-term care communities. This expansive footprint makes it possible to host student rotations across diverse regions, while maintaining the same integrated, outcomes-focused standards of care. Whether in urban centers or more rural communities, students gain the opportunity to learn in settings where their work makes an immediate impact on medically vulnerable populations.
Student placements are carefully matched to ensure that each rotation is meaningful, accessible, and aligned to academic requirements. Factors include:
To ensure a smooth start, students must complete all onboarding steps before their rotation begins.
3. Agreements: Students complete a DocuSign Student & HIPAA Agreement sent from Eventus WholeHealth.
4. Access & Orientation: EMR access is provisioned once agreements are signed. Students then receive a Welcome Email and an Onboarding packet connecting them with their assigned preceptor. Scheduling is coordinated directly between student and preceptor.
Our goal is to make the application and onboarding process seamless, giving students quick access to their preceptors and the tools they need for a successful rotation.
Eventus WholeHealth requires a Clinical Affiliation Agreement before any student placement, along with a copy of the school’s Certificate of Insurance. These agreements ensure compliance, outline responsibilities, and safeguard the quality of training for students and universities alike.
To request an affiliation agreement, please reach out to us at training@eventuswh.com or complete the request form [here]. Our team will guide you through the process to establish your school as an approved partner site.
Eventus welcomes students from NP, DNP, PA, and DO programs.
