Dr. Grumet’s heart lies in Family Medicine because she feels it sees a person’s health in the context of family, social and economic realities. Her practice has been caring for the elderly for the last 13 1/2 years, and is particularly proud of how much she has learned.

A Clinician Scientist and Physician, Dr. Grumet has specialty training and experience in Palliative and Hospice Medicine, Geriatric Medicine, and holds a Master’s Degree in Public Health.

Dr. Grumet serves as Chief Medical Director of Assisted Living Medicine for Eventus WholeHealth and looks forward to many years of serving Eventus clinicians, communities, and patients.

Cathy Parker is certified Gerontological Nurse Practitioner with a passion for serving those residing in long term care and assisted living settings. She graduated with a Master’s Degree in Nursing from University of Tennessee, Knoxville in 1999 and has been practicing in long term care ever since. Her passion with this environment dates back to her experience working as CNA during high school and college.

In 2005 she formed Extended Care Specialists alongside Susan Halley and became the trailblazers of providing integrated primary, mental health and wound care in Indiana in the long term care setting. In January 2020 they made the decision to merge with Eventus where their mission of serving residents in these settings has now expanded throughout neighboring states of Kentucky and Ohio.

In 2021 she developed an interest in the evolution of fee for service health care into the value based care model. She quickly found the emphasis on quality care, enhanced coordination of care, decreasing duplication of services and wise spending of health care dollars resonated with her vision of healthcare. She has since transitioned into the Director of Value Based Care for Eventus and has had proven success with the MSSP model over the past 3 years. She is committed to helping providers and facilities navigate and embrace the Value Based Care environment.

Dr. Patel’s career began in a traditional internal medicine practice and was comprised of office hours, hospital rounding and venturing into skilled nursing facilities. Within 2 years he realized that his passion was for post-acute and long-term care. He then went on to build a practice with a focus on skilled nursing facilities and assisted living facilities. During this time Dr. Patel also became medical director at various facilities and found that with the right mindset and approach to care in nursing homes, patients could indeed thrive. In his continued commitment to learning and to serving this patient population, Dr Patel became an engaged member of AMDA, the American Medical Director Association, and eventually received his CMD (Certified Medical Director) credentials.

As a result of his many years of experience as a Medical Director at various skilled nursing facilities, Dr. Patel has developed the unique understanding and expertise required to achieve superior outcomes. He leverages the power of a truly integrated approach to patient care and engages with facilities to improve quality metrics. Through education, mentorship and empowerment Dr. Patel leads a team of highly skilled primary care clinicians dedicated to caring for patients in the post-acute and long-term care setting. Together, they are committed to providing the very best in medical directorship, clinical care, facility engagement, and healthcare value.

Susan Halley is certified Nurse Practitioner with a passion for serving those residing in long term care and assisted living settings. She graduated with a Master’s Degree in Nursing from Indiana Wesleyan University .

With a passion for promoting the skills and capabilities associated with the Nurse Practitioner credential, Susan partnered with Cathy Parker in 2005 to form Extended Care Specialists. Employing Nurse Practitioners, Extended Care Specialists delivered primary care and mental health services to people residing in long term care. The success of Extended Care Specialists was the result of Susan’s commitment to introducing the value of nurse practitioners to the established community of physicians providing medical directorship and physician services in long term care.

Extended Care Specialists merged with Eventus in January of 2020 and Susan has remained a key member of the leadership team, currently responsible for clinical operations for the Midwest region. Susan is committed to excellent patient care, clinical leadership and creating a culture where Eventus is a destination employer for clinicians with a passion for providing care to the elderly.

Laura Hardcastle is a nurse practitioner who has specialized in both family medicine and psychiatric mental health. As a PMHNP Laura is trained in both medication management and psychotherapy and has applied her education and experience to providing care to the elderly for many years and has been a clinical leader on the Eventus team since 2014.

With a passion for delivering great care, Laura is utilizing her expertise and strong organizational skills to lead the clinical operations for the Southeast Region of Eventus. In addition, Laura is ensuring the entire Eventus organization is following Eventus proprietary mental health services best practices to ensure achievement of optimal outcomes.

Dr. Levine earned his PhD in Psychology and Organizational Development from Pennsylvania State University. His diverse career includes leading Psychology and Deafness Services for the State of North Carolina to Director of Human Resource Organizational Development at GlaxoSmithKline, to designing a program teaching up and coming chefs at the New England Culinary institute people management skills and ultimately to Eventus WholeHealth as a leader of mental health services and psychotherapy as well as being one of the Founders of the Company. Most recently Dr. Levine has applied his experience and interest to leading Eventus’ participation in clinical trials focused on a drug that is designed to reverse some of the symptoms of Alzheimer’s.

Dr. Levine is committed as well to helping make Eventus the employer of choice, building a culture of collaboration, support and professionalism. Dr. Levine takes pride in his ability to coach therapists on strategies for achieving optimal outcomes with patients. He shares his expertise and proven techniques regularly with Eventus psychotherapy providers to ensure the best possible outcomes are achieved for all Eventus patients.

Dr. Williams is a geriatric psychiatrist with more than 20 years of experience and earned his medical degree from the Brody School of Medicine at East Carolina University.

Founder and CEO of Geriatric Neuropsychiatry Services PLLC, Dr. Williams developed a successful practice providing psychiatric services to people residing in long-term care communities throughout North Carolina. With expertise in Alzheimer’s Disease, Lewy Body Dementia (LBD), Cerebellar Degeneration and Susac Syndrome, Dr. Williams and his team of providers were a great addition to the Eventus team when he merged his organization into Eventus in 2020.

As Director of Geriatric Psychiatry, Dr. Williams ensures all providers are thoroughly trained to deliver consistent care to produce the best possible outcomes.

Double board-certified in Psychiatry and Internal Medicine, Dr. Mezzullo provides a unique whole-person perspective to the care delivered by Eventus WholeHealth providers. Dr. Mezzullo has extensive experience providing care in both in-patient and out-patient environments including many years in leadership positions with the top North Carolina medical systems.

As Chief Medical Director of Behavioral Health for Eventus, Dr. Mezzullo helps train and lead the team of more than 250 Psychiatrists, PMHNPS, NPs and PAs. With an acute focus on quality and outcomes, Dr. Mezzullo is helping to ensure Eventus continues to build on a long history of leadership in Behavioral Health services to the Geriatric population.

As a dedicated Long-Term Care Physician, Dr. Dirk Juschka, best known as simply “Dr. Dirk” stands at the forefront of compassionate and comprehensive healthcare. With a passion for geriatric medicine and a commitment to enhancing the quality of life for older adults, Dr. Dirk brings a wealth of experience and expertise to the realm of long-term care.

Background: Dr. Dirk completed medical school at the University of Toronto in Ontario, Canada, and pursued family practice with increasing focus on geriatrics over the years.  This extensive educational journey laid the foundation for a career devoted to the unique healthcare needs of the elderly.

Professional Experience: With more than forty years of experience in the field, Dr. Dirk has become a trusted healthcare provider in the long-term care community. Having served in various capacities, from nursing homes to assisted living facilities, Dr. Dirk has honed a holistic approach that prioritizes both physical and emotional well-being.

Patient-Centered Approach: Known for a patient-centered approach, Dr. Dirk believes in fostering strong doctor-patient relationships. By actively listening to the concerns and aspirations of older adults, Dr. Dirk tailors care plans that address individual needs and promote a sense of dignity and independence.  His own journey with both parents passing in nursing homes due to Alzheimer’s and vascular dementia, gives Dr. Dirk a unique personal perspective of patients in long term care.

Advocacy and Research: Beyond clinical practice, Dr. Dirk is passionate about advocacy and research in geriatric medicine. Actively participating in initiatives aimed at improving long-term care standards, Dr. Dirk strives to contribute to the evolving landscape of elderly healthcare.

Community Engagement: By sharing knowledge and insights, Dr. Dirk empowers families and caregivers to make informed decisions regarding the well-being of their loved ones.

Personal Touch: Outside of the medical realm, Dr. Dirk enjoys spending time with his children and new grand baby, finding a personal connection with patients through shared interests and experiences. Dr. R. Steven McDonald, DO, MSMI, is leading our organization into a future where healthcare meets cutting-edge technology. As our CIIO, Dr. McDonald spearheads strategic technology initiatives, ensuring our organization remains at the forefront of innovation. Dr. McDonald is dedicated to optimizing the use of medical information. Dr McDonald earned his undergraduate degree from Michigan State University before pursuing his DO from Michigan State University College of Osteopathic Medicine. Dr McDonald rounded out his educational journey with a Masters in Medical Informatics from Northwestern University.

As a dedicated healthcare professional, Kimberley Brumfield has been at the forefront of long-term care nursing for over two decades. She earned her BSN in 1992 and her MSN and FNP in 1999, marking the beginning of a passionate and impactful career in the field.

Professional Background: Kimberley’s expertise lies in the nuanced and essential realm of long-term care. She has consistently demonstrated a commitment to providing compassionate, patient-centered care that goes beyond routine medical attention. With a keen understanding of the unique needs of the aging population, Kimberley has become a trusted advocate for her patients, ensuring their comfort and well-being are prioritized.

Educational Achievements: Graduating with distinction, Kimberley completed her Master’s in Nursing with a focus on advanced practice. This educational foundation has empowered her with the knowledge and skills necessary to navigate the complex landscape of long-term care, blending clinical acumen with a deep understanding of the human aspect of healthcare.

Passion for Long-Term Care: Kimberley’s passion for long-term care extends beyond the clinical setting. She actively seeks to contribute to the broader conversation surrounding geriatric health, advocating for policies that enhance the quality of life for elderly individuals. Her commitment to ongoing learning and staying abreast of the latest advancements in healthcare underscores her dedication to providing the best possible care.

Community Engagement: Recognizing the importance of community involvement, Kimberley has engaged in outreach programs aimed at educating both healthcare professionals and the general public about the challenges and opportunities in long-term care. Through workshops and informational sessions, she strives to foster a greater understanding of the unique healthcare needs of older adults.

Monica Leriger is a Board-Certified Geriatric Pharmacist who helps prescribers assess drug safety and minimize adverse outcomes. Monica has worked for 20 years in long-term care as an educator of antibiotic stewardship and deprescribing. She is an adjunct faculty at Butler University where she precepts PharmD candidates in geriatrics with an emphasis in psychotropics, antibiotic stewardship, and polypharmacy. After successfully leading her facilities through COVID treatments, Monica has turned her focus to raising awareness of medications negatively impacting CMS quality measures.

Monica enjoys travels throughout the United States, visiting forty-eight states with her family, and looks forward to more international travel after reach state #50.
